By signing this Consent/Release Form, I acknowledge that camps, trainings, programs, activities and trips conducted by Inspire Global Network, and its related and affiliated organizations (collectively, Inspire Soccer FC), especially, but not limited to, day camps, overnight national and international camps and trips, and gap years abroad (individually or collectively, the camps, trips and activities organized by Inspire Global Network) involve a certain amount of risk to individuals participating. Activities may include strenuous or moderately strenuous physical activities including, but not limited to:

Soccer Specific Activities:

competitive matches and scrimmages, small-sided and full-field games, position-specific drills (shooting, defending, goalkeeping), high-intensity tactical and technical training, repetitive kicking, passing, and ball control drills, heading the ball;

Conditioning & Fitness Training:

sprinting, interval running, and endurance running, agility drills (ladder drills, cone drills, change of direction), plyometric exercises (jumping, bounding, explosive movements), strength and bodyweight training (lunges, squats, push-ups, core work, speed and acceleration training, warm-up and cool-down routines;

Physical Contact & Movement:

physical contact with other players, rapid changes in direction and pace, jumping, landing, and aerial challenge, risk of falls, collisions, or being struck by a ball;

General Physical Activity Risks:

fatigue and overexertion, muscle soreness, strains, or sprains, slips, trips, and falls on indoor or outdoor surfaces;

Environmental & Heat-Related Exposure:

outdoor training in high temperatures and humidity, prolonged sun exposure, activities that may increase risk of dehydration or heat-related illness (e.g., heat exhaustion, heat stroke)

I understand that these activities involve inherent risks, including but not limited to, physical injury, heat-related illness, and other unforeseen conditions, and I voluntarily assume all such risks associated with participation. With this information, I acknowledge and consent to my or my minor child's participation in the camp(s) for which they have enrolled. I further acknowledge and consent to any organized travel by van, bus, or other transportation as part of the activities conducted during the camp(s) if any of the activities require such transportation.

Acknowledging the activities involved, I hereby release and hold harmless Inspire Global Network, its agencies, related organizations, employees, directors, counselors, leadership, volunteers and members from any and all liability that may arise as a result of my or my child's participation in the camps except in the case of gross negligence or intentional misconduct.

Medical Consent for Treatment

During the Inspire Global Network activities in which I or my child are participating, I hereby authorize the camp coordinators, adult camp director(s) and/or adult coaches designated to lead that camp or activity to consent to any necessary examination, anesthetic, medical diagnoses or hospital care for my minor child named below. I understand that I will be notified as soon as possible in the event of an emergency. I also understand that designated volunteer first responders may or may not be professionally trained doctors or nurses and may offer First Aid in the event of an accident.
